    The IL Valley Center for Independent Living – IVCIL

    Non-profit and advocacy organization that serves people with disabilities and helps them become more independent. Services include: information and referral, independent living skills training, peer counseling, and advocacy. They also provide programs such as: personal assistant, community reintegration, youth and deaf services, and a volunteer program. Also includes a resource library, equipment closet, and community services.

    Stepping Stones of Rockford

    Private, not-for-profit organization that provides housing and rehabilitation to persons with serious mental illness. Serves adults and their families by providing intensive care management and psychosocial rehabilitation for each individual’s needs. Also provides community education and advocacy on behalf of people with serious mental illnesses.

    Shelter Care Ministries

    Emergency and transitional housing for families and provides outreach and advocacy for people who are homeless and those who have mental illnesses. Also focuses on the needs of homeless families and adults with mental illnesses.

    RocVale Childrens Home

    Offers individualized progaramming for children and young adults with disabilities ages 6-21 including residential, medical, and behavioral and social services. Also operates Maguire and Forbes Group Homes for developmentally disabled adults.

    ResCare Homecare – Rockford Branch

    Provides patients with companion and housekeeping services, hospital support and care, medication reminders, new baby care and support, personal care and support, post hospitalization care and support, remote monitoring support, respite care, skilled nursing, and suport and care in assisted living facilities.

    Pine Acres Rehabilitation and Living Center

    A nursing facility offering short term rehabilitation or long term stays.

    Pay-It-Forward house

    Community Center. No residency Requirement. Open 8 a.m. to 9 p.m.

    PA Peterson Center for Health

    Provides independent living or sheltered care, short-term rehabilitation, outpatient therapy, community fitness program, nursing care services, programs for people with Alzheimer’s disease and dementia, and hospice and respite care.

    Open Door Rehabilitation Center

    Serves adults with developmental disabilities, such as mental retardation, Down’s Syndrome, cerebral palsy, and autism. Services include: sheltered workshop, vocational training, developmental training program, supported employment, community employment, supported living, and residential facilities.

    JDWI Foundation

    The JDWI Foundation provides grant funding for organizations and individuals who reside in Jo Daviess County to deal with issues of health, housing, transportation and/or employment. Applications and grant guidelines can be obtained by written request by mail or e-mail.
